Primary Care Physician
Houston, TXWe’re working on behalf of a premier, home-based healthcare provider and medical services company that is highly regarded for its innovative, multi-specialty practice. Experiencing tremendous growth nationwide, the company’s focus is comprehensive care at home to improve outcomes and reduce total healthcare costs. They are a group of clinical professionals that understand the needs of patients, medical professionals, health systems and payers.
We seek a Texas licensed MD or DO with Board Certification in Internal Medicine or Family Medicine for the role of In Home Primary Care Physician. The person in this role will perform preventive visits daily to optimize chronic conditions, assess home environment, and develop proactive care plans. He/she will visit 4-6 patients per day in their homes within a 45-mile radius of the Houston city center. Company vehicle is provided. He/she will educate patients and/or patient’s family on chronic medical conditions, preventative care, and medication adherence compliance. He/she will leverage the support of care team such as: nurse care manager, behavioral health, social work, and pharmacy to meet patients’ medical, biopsychosocial, and financial needs. He/she will complete EMR documentation (Epic) in a timely manner.
Candidates must have an MD or DO and hold Board Certification in Internal Medicine or Family Medicine. Preference will be given to candidates with Geriatric Medicine subspecialty training or experience. Must have valid state driver’s license. Must be a highly motivated, self-directed professional with strong organizational skills. Must be comfortable working independently on a daily basis. Must have a strong background/interest in primary care and preventative medicine.
The role offers competitive compensation (base salary - $230,000 to $250,000 based on experience), 15% bonus target, and target of $25,000 per year in restricted stock plans/options.
