RN Nurse Manager | Long Term Care

We're working on behalf of a 320-bed skilled nursing facility located in Briarwood, Queens.  The facility is a unique center of excellence dedicated to providing services to complex acute residents on ventilators and boasts New York State’s largest, nationally recognized and award winning respiratory/ventilator dependent care program. The facility also offers subacute/post-acute care services and in-patient hospice long term residents.  All patients, whether inpatient rehabilitation, outpatient rehabilitation or long-term care, are served well by excellent clinical staff - board certified physicians, pulmonologists, respiratory therapists, nurse practitioners, nurses, physical therapists, occupational therapists and speech pathologists.  All patients also enjoy a facility that is bright and accessible with special public places that they and their visitors are welcome to explore.

Looking for an opportunity with a leader?  We seek a Registered Nurse with two (2) or more years of LTC and/or Acute Care management experience for the role of RN Nurse Manager - Clinical Care Coordinator.  In this role you will be responsible for providing and overseeing complete resident assessment, care planning, patient, and caregiver education. You will evaluate progress toward goals and make recommendations for changes.

Candidates must have a Bachelors degree in Nursing and be licensed as an RN in the state of New York (or have the ability to gain this licensure).  Preference given to candidates with a Masters degree. Our ideal candidate must have at least two (2) years of supervisory experience as a registered nurse, in a long term care facility, and have working knowledge of Sigmacare (electronic medical record). Preference given to candidates with ANCC Gerontological certification. Knowledge of MDS, PRI and Care Planning is essential. Must be able to set goals and work in a self-directed fashion. Must be able to deal professionally and effectively with employees and residents. Must have strong writing, communication, and computer skills.
