Looking for a change of scenery? We seek a Registered Nurse with five (5) or more years of clinical nursing experience for the role of RN Case Manager. In this role you will utilize an interdisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care, focusing on complex medical and psychosocial needs of the Hospital’s patients and their families. You will perform pre-admission screening review of patients. You will assess and plan interdisciplinary plan of care that effectively manages patient along the continuum. You will ensure appropriate use of resources and coordinates discharge planning.
Candidates must have a Bachelors degree in Nursing and be licensed as a Registered Nurse in the state of Massachusetts (or have the ability to gain this licensure). Must have at least five (5) years clinical nursing experience. Must have a minimum of one (1) year of Case Management experience. Strong preference given to candidates with Case Management Certification. Preference given to candidates with working knowledge of InterQual Criteria. Midas and MediTech experience is also a big plus.
