We seek candidates with a Masters educated Social Worker with Pennsylvania LCSW certification for the role of Licensed Clinical Social Worker - Geriatric. The person in this role will be responsible for utilizing evidence-based therapeutic models within the context of a multi-disciplinary team to ensure the delivery of quality behavioral health services to patients and families. You will provide behavioral health support, education, and supervisory services to colleagues and to other providers and agencies as needed.l You will work with Geriatric patients ages 65 and up. You will counsel on your own as well as extensive visits to asses physical/mental issues. You will coordinate the Senior Assessment Team to include a collaborative model of assessment, treatment, and follow-up between the psychiatrist and geriatricians, with primary care physicians, case management, social work, nursing and other providers. You will be responsible for community outreach, marketing and education.

Candidates must have a Masters degree in Social Worker and hold LCSW licensure.  Must be able to develop the work flow and oversee day-to-day operations including triaging and scheduling assessments for Senior Assessment Program. Must be able to perform comprehensive assessments and provide quality psychotherapy utilizing evidence-based practices. Must be able to act as a resource for older adult issues including behavioral health, physical health, caregiver concerns, and maintain knowledge of community
resources.
