We’re working on behalf of a healthcare leader that offers specialized medical care focused on addressing a patient’s specific symptoms, pain, and stress; and improving quality of life for both patients and their families. Working together with a patient’s medical team, the organization’s clinicians develop an integrated care plan to help manage symptoms such as pain, shortness of breath, fatigue, nausea, loss of appetite, difficulty sleeping and depression. The company also offers 24/7 support to patients, including nurse practitioner home visits.
We seek candidates with a Master of Science and Nursing and previous experience in home-based services, complex cases, and/or palliative care for the role of Nurse Practitioner. In this role you will collaborate with Lead Physicians, the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values. You will take part in weekly interdisciplinary team meetings, call schedules and scheduled in-service and education sessions.
Candidates must have a MSN and licensed in the state of Indiana (or have the ability to gain this licensure). Home health/hospice, complex cases, and/or palliative care experience is preferred. Acute care bedside, oncology, ER or ICU experience is a plus. Possession of DEA registration or eligibility required. CPR certification required. Must have a reliable automobile, valid driver’s license, and minimum state required liability auto insurance (reimbursements are made for mileage as you travel in personal vehicle to scheduled appointments). Must have excellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful.
