Clinical Pharmacy Specialist - Transplant

Through evaluation, pre-surgery management, surgery, postoperative care, and beyond, transplant patients have a highly skilled and empathetic team of experts deeply committed to improving their health and quality of life. The healthcare system is one of only a few in the region with the expertise to handle a wide variety of patients eligible for kidney transplant, liver transplant, hepatobiliary surgery, and medical management of liver diseases.
 
We seek candidates with a PGY-2 in Organ Transplant for the role of Transplant Clinical Pharmacy Specialist. The person in this role will coordinate and provide pharmaceutical related care to transplant patients and will provide leadership to pharmacy staff pertinent to associated job responsibilities. He/she will provide comprehensive pharmaceutical care to transplant recipients in a culturally competent manner to include, evaluation, identification and solution of medication related problems for transplant recipients. He/she will prepare and assist with discharge planning for all transplant recipients. He/she will evaluate transplant recipient medication regimen on a regular basis. He/she will review, interpret and evaluate medication orders, labs, and other clinically relevant information for appropriateness. He/she will assure timely order verification and distribution of pharmaceuticals.
 
Candidates must have a Pharm.D. and be licensed as a pharmacist in the state of Pennsylvania.  Must have two (2) or more years of experience treating transplant patients in an acute care setting.  Must have PGY-1 and PGY-2 residency experiences.
