We’re working on behalf of a healthcare organization that administers post-acute health care services through more than 760 home nursing agencies, hospices, and long-term acute care hospitals. Their home health nursing agencies provide care to Medicare beneficiaries, offering such services as private duty nursing, physical therapy, and medically-oriented social services. Its hospices provide palliative care for terminal patients, while its LTACHs serve patients who no longer need intensive care but still require complex care in a hospital setting.  Today the organization ranks among the top 3 home health providers in the US.

We seek a Texas licensed RN with skilled home health experience including Medicare OASIS admissions for the role of RN Patient Care Manager. In this role you will receive referrals and ensure appropriate clinician and/or therapist(s) assignments for timely patient evaluation by signing off after authorization and plotting start of care (SOC) visits. You will coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals. You will ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance to physician orders. You will oversee and assure development, implementation, and updates to the individualized patient plan of care, as appropriate. You will manage and document phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians. Uses coordination notes to document, as needed and appropriate.

Candidates must be a graduate of an accredited school of nursing and be licensed as a Registered Nurse in the state of Texas. Must have two (2) or more years of home health nursing experience and experience with Medicare OASIS admissions. Must have current CPR. Must have current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
