We seek a West Virginia licensed RN with skilled home health experience including Medicare OASIS admissions for the role of RN (Home Health). In this role you will provide clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. You will make the initial and|or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care, and submit accurate, complete, and timely documentation, per policy. You will regularly evaluate and re-evaluate (as warranted by changes in condition but at least every 60 days) the patient's nursing needs. You will perform patient comprehensive assessments at designated time points and develops the appropriate POC, in collaboration with physician orders.
Candidates must be a graduate of an accredited school of nursing and be licensed as a Registered Nurse in the state of West Virginia. Must have one (1) or more years of home health nursing experience and experience with Medicare OASIS admissions. Must have current CPR. Must have current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
