We seek a Registered Nurse with inpatient case manager experience for the role of RN – Case Manager. In this role you will evaluate the quality, care, treatment, and outcomes of complex patient populations. You will collaborate with the patient/family and caregivers to facilitate necessary resources and options for post-discharge services to support a safe discharge. You will provide consultation with physicians and other members of the interdisciplinary healthcare team to facilitate patient progression to discharge. You will ensure clinical documentation supports medical necessity criteria and meet regulatory requirements. You will communicate the clinical status of patients related to medical necessity criteria to the physician advisor (internal or external) and/or the Inpatient Care Transitions leadership team as needed.
Candidates must have a Bachelors degree in Nursing and be licensed as an RN in the state of Indiana (or have compact state licensure). Strong preference given to candidates with CCM certification. Must have at least two (2) years of experience in an acute care setting. Experience or knowledge of InterQual criteria is a big plus. Must demonstrate knowledge of DRGs managed care initiatives payment methods and statistical analysis. Must be able to engage in complex clinical decision-making.
