We seek a Registered Nurse with at least one (1) year of home health nursing experience for the role of RN – Home Health. The person in this role will implement and maintain established department, unit, administrative, and clinical policies/procedures for patient care, safety, and infection control. She/he will participate in improving organizational performance activities as assigned. She/he will conduct initial and ongoing patient assessment with attention to the patient's physical, emotional, psychosocial, functional, home environment, and spiritual needs. She/he will develop and implement individualized plan of care for each patient. She/he will provide care to patients in the home and consults with other specialists as indicated.
Candidates must be a graduate of an accredited school of nursing and be licensed as a Registered Nurse in the state of California. Must have one (1) or more years of home health nursing experience. Must have current California driver’s license with auto insurance coverage. Must be able to work independently.
