We’re working on behalf of a healthcare organization that provides in-home and telehealth care services to serve the diverse needs of patients and their partners. The company has focused their business on providing coordinated, in-home patient care for over 25 years. They collaborate with patients, doctors, health plans, and other groups to manage diverse and complex populations and identify and address barriers to care, enhancing the quality of life and improving outcomes. 

We seek candidates with previous experience working with the geriatric population for the role of Field Based Registered Nurse. The person in this role will be primarily responsible for conducting in home visits, nursing assessments, patient triage, care coordination, and patient/family education for the members assigned to the provider. She/he will support providers and their respective patients, and also address client-facing needs per POD/market. She/he will perform home visits with patients enrolled in the organization’s Chronic Risk Program and/or Advanced Care program (ACP).

Candidates must be a graduate of an accredited school of nursing and hold active, unrestricted RN license in Tennessee. Must have three (3) or more years of experience as a Registered Nurse that includes home health nursing experience. Preference given to candidates with previous experience working with the geriatric population/ chronic condition experience. Case management experience is a plus. Must be a highly organized, self-directed worker with an ability to function in high volume environment. Must have strong verbal and written communication skills. Prior clinical experience in palliative care, end of life, hospice, oncology, ICU, geriatrics is preferred. Knowledge of STARS and Hedis metrics a plus.
