We seek a California-licensed MD or DO for the role of Primary Care Physician. The person in this role will perform in-person comprehensive assessments and coordinate with the IDT to develop a care plan for each participant. At the direction of the Medical Director, he/she will oversee all care of participants in nursing homes and hospitals including admits and discharges for acute, short-term, and long-term placement and the use of medical specialists to ensure effective care coordination. He/she will also provide direction and support to Nurse Practitioners and other members of the IDT.

Candidates must have a MD or DO and be licensed in the state of California without restrictions. Must hold Board Certification in Internal Medicine or Family Practice.
