We seek a California LCSW with two (2) or more years of acute care case management experience for the role of Care Manager LCSW. The person in this role will join a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. She/he will facilitate communication and care coordination along a continuum through effective transitional care management. She/he will recognize the patient's right to self-determination, the significance of the social determinants of health, and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

Candidates must have a Master's Degree in Social Work from an accredited program and hold California Clinical Social Worker licensure upon hire. Must have two (2) or more years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
