Physical Therapist

We’re working on behalf of a healthcare organization whose PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities. They employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to their values, team culture, and mission.

We seek candidates with two (2) or more years of experience with the frail and/or elderly poluation for the role of Physical Therapist. The person in this role will conduct Physical Therapy assessments in the facility and in the home to define the Physical Therapy care needs, preferences, and goals of the participants. He/she will actively participate in IDT meetings to develop participant care plans. He/she will deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans. He/she will provide active and passive exercise, muscle re-education, gait, and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity. He/she will supervise the Therapy Assistants, including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP). He/she will provide training and instructions to staff, participants, and caregivers/families in the use and care of therapeutic equipment.

Candidates must have a Bachelors degree in Physical Therapy from an accredited program and hold valid California PT License, or proof of License eligibility. Master’s Degree in Physical Therapy or Doctorate in Physical Therapy preferred. Must have at least two (2) years of OTA experience in a clinical setting. Must have the requisite knowledge to treat frail, elderly participants with complex conditions.
